Breaking brakepads

Ok here`s the problem:

for the second time in a short while the fibre (?) brakepad closest to the center gearbox (dual disk brakes) has broken in 2 and slided off the metal pad with the result that I cannot brake anymore because there can`t be enough brakepressure. So I have to take half the truck apart again to replace the broken fibre brakepad.... sigh....

has anyone else had this problem and possible a solution?
 
I have not had this problem but what I did when I replaced my pads I attached the fiber pad to the metal with JB weld and clamped it over night I have run 4 gallons through the truck since then and not had any problems with the brakes
 
1) Make sure the metal plates are very clean when you stick the pads on them.

2) Stack up the plates and clamp them very tightly in a vise to seat them.

3) Remove them from the vise and glue the edges of each individual pad and plate with some C/A "Cyanoacrylate" glue. (Just the edges).
