This is what happened to my trans when my cb screw backed out on me. After inspecting everything the result was from me being a noob and forgetting to put a dab of threadlock on it.

actually i dont think you need thread lock on the cb screw. i dont use it anyways. i used to but bank ( i think) steered me clear of it and i haven't went back. Crazy carnage tho!
